Meeting Change Request

If you want to change your meeting information:

  • Fill out the form below
  • Be sure to fill out all required fields marked with an asterix (*)
  • Be sure to include the changes you want to make in the text box under the text “Please list your changes here (be specific):”
  • Click on the “submit” button

You will be sent a confirmation email.
